Neighborhood Health, Reach Out and Read, and Social Cascade Announce New Partnership

Social Cascade is thrilled to announce a new partnership with Reach Out and Read in Virginia and Neighborhood Health. A leading provider of quality healthcare services in Virginia, Neighborhood Health's mission is to improve health and advance health equity by providing high-quality primary care regardless of ability to pay.

Our partnership with Neighborhood Health will allow us to combine our respective strengths and resources to offer a broader range of services to patients and families. Together, we will work to address the unique healthcare needs of the community and ensure that everyone has access to quality healthcare and the critical information needed to empower families to make informed decisions about their health.

As part of this partnership, we will be working to:

• Expand access to critical care information through accessible, ubiquitous communication channels
• Reinforce messages delivered during primary care appointments throughout the year where patients are already looking for information
• Improve awareness of local resources and services that address social determinants of health
• Build health literacy and empower health-promoting behaviors

Reach Out and Read is a nonprofit with a mission to strengthen all families with young children through guidance from medical clinicians about nurturing relationships through shared reading. The regional office serves children from birth to five years old in North Carolina, South Carolina, Virginia, DC, and Maryland.
